1. Request the Permit: The person responsible for the work logs a permit request, defining what needs to be done, where, when, and by whom. This creates a structured record that moves through the approval process.
2. Identify the Risks: Hazards are assessed based on the work type and site conditions. A Job Safety Analysis is completed, and the controls required to proceed safely are documented before anything moves forward.
3. Confirm Isolation: The equipment and energy sources involved are locked out and tagged (LOTO). Isolating authorities verify that zero-energy state has been achieved and attach proof to the permit record.
4. Review and Issue: Designated approvers (EHS managers, area owners, or other stakeholders) review the complete package: work scope, risk assessment, and isolation confirmation. When satisfied, they issue the permit.
5. Execute the Work: Work proceeds within the conditions defined in the permit. Controls stay in place, and site activity is monitored throughout.
6. Clear and Restore: Often called de-isolation or hand-back, this is the critical, final phase of a maintenance or repair job. It is the formalized, step-by-step process of safely returning equipment or systems from a locked-out, zero-energy state back to normal, active operations.
7. Close the Permit: All parties sign off. The permit is formally closed and the complete record, i.e., every action, approval, and verification, is retained as a permanent audit trail.

This sequence applies across industries such as manufacturing, oil and gas, utilities, construction, pharmaceuticals, and facilities management. What varies is the weight each step carries. In high-hazard environments, isolation and verification are the most scrutinized stages. In others, the risk assessment bears the most regulatory load. The steps remain the same; the depth of each one is determined by the work.
